Late Cancellation -A $25 fee applies to appointments canceled after 7:00 AM on the day of the appointment. If you plan to use insurance, please verify that we are in-network with your plan before your visit. Late cancellation fees will apply if you cancel your appointment on the day of your exam because we are out-of-network with your insurance or your insurance does not cover the exam.
